Electronic Surveys

All surveys for the 2013 PIT count are listed below.  It is highly recommended that you download and review all surveys relevant to your program and/or agency before the count.

General Intakes

Housing Type #1

Housing Type #2

For those that would prefer a paper-based General Intakes survey, please click the link below.  Please note: If you do download the paper-based survey, transfer all paper surveys to an electronic survey following the count, and submit the electronic survey.  If you do not have the capability of using the electronic survey, please contact your PIT coordinator.

What Surveys Should I Use?

All programs or agencies should use the General Intakes survey.

The following type of housing programs should fill out Housing Type# 1:

  • Permanent Supportive Housing
  • Rapid Re-housing

The following type of housing programs should fill out Housing Type#2:

  • Emergency Shelter
  • Transitional Housing
  • Safe Haven

If you are still unsure, please contact your Continuum of Care’s Point-in-Time Count Coordinator.
